Liability Waiver, Assumption of Risk, and Media Release

By signing below, I acknowledge and agree to the following:

Assumption of Risk

I understand that participation in Brazilian Jiu Jitsu, Muay Thai, martial arts training, strength and conditioning, sparring, drills, competitions, open mat sessions, and related activities involves inherent risks, including but not limited to:

• Serious bodily injury

• Cuts, bruises, sprains, fractures, or broken bones

• Concussions or head injuries

• Permanent disability

• Illness or infection

• Paralysis or death

I voluntarily choose to participate in activities at Double Black Jiu Jitsu LLC and fully accept all risks associated with participation, whether known or unknown.

Release of Liability

I hereby release, waive, and discharge Double Black Jiu Jitsu LLC, its owners, coaches, staff, volunteers, affiliates, members, and facility providers from any and all liability, claims, demands, actions, or causes of action arising from injury, illness, loss, or damage related to participation in any activities at or associated with Double Black Jiu Jitsu LLC.

This release includes claims arising from negligence to the fullest extent permitted by law.

Medical Responsibility

I understand that I am responsible for determining whether I am physically and medically capable of participating in training activities. I agree to immediately notify staff of any injury, medical condition, or concern that may affect participation.

I authorize emergency medical treatment if necessary and understand that I am solely responsible for any related medical expenses.

Hygiene and Safety

I agree to follow all gym rules, safety standards, hygiene requirements, and coach instructions while participating in training activities.

I understand that failure to follow safety or hygiene standards may result in suspension or removal from training.

Photo and Media Release

I grant Double Black Jiu Jitsu LLC permission to photograph and record me and/or my child during classes, events, competitions, or related activities.

I understand that these photos or videos may be used for:

• Social media

• Website content

• Advertising

• Promotional materials

• Educational or marketing purposes

I waive any right to compensation, ownership, or approval regarding the use of such media.

Minor Participant Agreement

If signing on behalf of a minor participant, I certify that I am the parent or legal guardian of the participant and agree to all terms on their behalf.

I understand and accept all risks associated with participation and authorize participation under this agreement.
